Carol A. Addy, PA-C
After eleven years of experience as an RN including ER, ICU, Internal Medicine and Surgery, Carol graduated with honors from the University of Texas Southwestern in Dallas, where she received her PA degree. She is Board Certified in Family Medicine.
She is certified by the National Commission on Certification of Physician Assistants, and is a member of the American Academy of Physician Assistants and the Texas Academy of Physician Assistants.
Carol enjoys all aspects of family care and has provided care to many of her patients for over 20 years.
Carol stays busy playing with her grandchildren, being with family and friends, traveling and participating in medical mission trips to Nicaragua.
David C. Yi, M.D.
Dr. David Yi graduated from Johns Hopkins University with a degree in Biophysics and went to the University of Texas Southwestern Medical Center. He completed his residency in Family Medicine at John Peter Smith Hospital in Fort Worth. He then practiced as a hospitalist for two years before joining Trophy Club Family Medicine in 2006. He is Board Certified in Family Medicine.
He believes strongly in looking deeper into the root cause of disease and disability and not just symptom management. He actually likes to care for patients with complex issues as he finds it rewarding to make complex problems manageable and simplify treatment and medication regimens.
Dr. Yi and his wife are active in their church and often go on medical mission trips to Belize.
Dr. Thomas C. Diaz, D.O.
Dr. Thomas C. Diaz is a native of West Monroe, Louisiana. He earned a Bachelor of Science in Biology and a Bachelor of Arts in Spanish from Ouachita Baptist University. He attended medical school at William Carey University College of Osteopathic Medicine and completed his residency at Medical City Fort Worth in Fort Worth, Texas.
Dr. Diaz is a Board-Certified Family Medicine Physician who believes in a holistic approach to health, with an emphasis on preventative care. During his residency, he completed an academic tract in Osteopathic Manipulative Treatment (OMT) and frequently utilizes OMT as a non-invasive approach in the management of chronic pain. He is bilingual, fluent in both English and Spanish.
Dr. Diaz enjoys spending time with his wife, going on hikes, and lifting weights.
